Azrar Ayub

Azrar Ayub was just 27 years old when he died following a restraint at the Edenfield psychiatric unit of Prestwich Park Hospital in Bury. During the last few weeks of his life Azrar had been subjected to an increasing number of restraints resulting in injury. An Inquest jury heard how on the 28th May 2004 Azrar was subjected to restraint having been injected with the maximum dose of lorazapam. The restraint lasted for about 25 minutes by which time Azrar was exhausted. Azrar was left face down on a mattress in a seclusion room where he died. The effects of the restraint and the injection meant that he was unable to move. The jury found that the NHS policies in place at the time of Azrar’s death were insufficient and that individuals had breached policies. The jury also found that the staff had attempted to conceal what had transpired in relation to the kneeling on Azrar’s back, carrying him and their lack of observations of him.
